The following form is intended to assist you in requesting a Certificate of Insurance (COI) from your insurance company.  Simply fill out the form below and fax it to your insurance company.  They will forward your current Insurance Certificate to the POC office.

If you prefer not to use this form to request a COI from your insurance company, please ensure that the following requirements are met:

BODILY INJURY AND PROPERTY DAMAGE

Aggregate coverage of $1,000,000 bodily injury and $200,000 property damage.

ADDITIONAL INSURED (BOTH MUST BE NAMED ON CERTIFICATE)
